**Specify you are scheduling SC Sports Physical
Laporte Center will be scheduling appointments for all school students that require a pre-participation sports physical exam. All appointments need to be scheduled ahead of time. We cannot accept walk-in appointments.
Please note the following information that will be required on the day of the appointment.
1. Students must arrive 15 minutes prior to the appointment time.
2. Students must be accompanied by a parent or legal guardian to their appointment, otherwise the appointment will need to be rescheduled.
3. Parents must complete and sign all sports physical forms prior to the appointment and bring them to the appointment, otherwise the appointment will need to be rescheduled.
4. You may elect to see your own physician.
No Physical= No Practice, No Exceptions!
